By clicking above, I agree that I have been vaccinated against Covid-19, and that I will not attend the performance if I test positive for Covid within 72 hours of the performance. In addition, I will not attend the performance if I am experiencing Covid-19 symptoms. I will bring proof of vaccination to the venue, and I will wear a mask for the entire time I am inside the venue.

COVID-19 Waiver

TENET Vocal Artists (“TENET”) has taken all reasonable steps, consistent with public health guidelines announced by the U.S. Centers for Disease Control and Prevention and New York State and City public health authorities, to reduce the spread of COVID-19 during our activities, but TENET cannot guarantee that while participating in rehearsals or performances or attending concerts a person will encounter no risk of infection.  I understand that attending public concerts may expose me to the risk of infection.  Therefore, by clicking the button below, I agree to assume the risk that my participation may expose me to the virus and I accept sole responsibility for any consequent injury to myself, illness, damage, loss, claim, liability or expense, of any kind, that I may experience in connection with my attendance at a TENET concert (“Claims”).  On behalf of myself, I hereby release, promise not to sue, discharge, and hold harmless TENET, its employees, agents, and representatives from any Claims arising out of or relating to my attendance at a TENET concert.  I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of TENET, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after my attendance at any TENET concert.
